How to register as a new patient

Registering as a new patient with Limelight Health and Wellbeing Hub has never been easier.  All you need to do is complete our online pre-registration form and we will do the rest. 

We are only able to accept patients residing in the M15 and M16 postcode.

The information you will need to register:

You will be asked to provide us with some personal details such as your:

  • Name
  • Address
  • Date of birth
  • Previous address
  • Previous GP

Once your registration is complete, you will receive a welcome text message informing you that you have now been successfully registered and details of how you can set up an account for our ASKMYGP service.

Non-urgent advice: What if you have missing information

We ask that you provide us with as much information as possible on your pre-registration form to register you when we receive your request. This ensures that your previous medical records are transferred over to us to ensure continuity in your care.

If you do not have the information to hand or there is information missing for us to complete your registration, don’t worry; our reception team will contact you to complete your registration.

If you are unable to complete the online pre-registration form, you can visit our reception desk and complete the paper registration form. Our reception is open from Monday to Friday 8am to 6.30pm.
